Food Vendor Licensing
Dear Dedicated Food Vendor!
Welcome to the Raw Spirit Festival!
Please understand that these governmental regulations are beyond our control. We are happy to answer any questions you may have about this process and to assist you in any way within our power to comply with these lovely licensing requirements:
Fabulous Food Vendors, please take note:
The Raw Spirit Festival is required by law to request the following two tasks be done by all of our amazing food vendors:
1. Licensing with Cococino County
2. Sending insurance certificates to Raw Spirit Festival for our records
Thank you in advance for your prompt attention to these details! Please know we truly appreciate you and all your efforts and your presence at the festival, making it such a wonderfully delicious and diverse experience for our many hungry participants!
1. FOOD VENDOR LICENSE WITH COCOCINO COUNTY
Since the Festival will be held in Coconino County, Arizona, the Vendor/Sponsor accepts full responsibility for understanding, obtaining, paying for and displaying either the $65 (food sample giveaways) or $95 (food vending) event license options as explained at:
Should food vendors not comply with this, the food vendor may be fined by the county or be required to close their booth.
VENDORS SELLING PREPACKAGED FOODS
If prepackaged foods remain in the package, and are sold in the sealed package, there is no need for a license. However, if the food vendor who has packaged foods is going to be giving out or selling samples from packages that have been opened, they will need the license to be able to be at the festival.

2. YOUR INSURANCE FOR FOOD PREPARATION FOR THE PUBLIC
a. The Raw Spirit Festival Inc. is required by law to have a copy of your insurance certificate, sent directly from your insurance company to our offices so we can keep it on file.
b. Please also be sure to bring a copy to the festival itself.
c. Please have your insurer state in writing the following, and include this written statement with the Certificate of Insurance:
"This Certificate of Insurance indemnifies the Raw Spirit Festival Inc., it's employees, volunteers and the Venue, the Radisson Resort, and it's employees. The Raw Spirit Festival takes place September 12-14th, 2008 at The Radisson Resort, Sedona, Ariozna."
d. Please have your insurance company mail a copy of your certificated of insurance for preparing food for the public to:

Special Events/Temporary Food Establishments
A Temporary Food Establishment is an operation that prepares and serves food in a self-contained mobile unit or a tent.
Temporary Food Service operations at special events must abide by the Coconino County Food Code. For a list of requirements please refer to the 2006 Temporary Food Service Requirements for Special Events. Failure to comply may result in closure.
To apply for a License to Operate you must submit theTemporary Food Service Application to the Environmental Health Office, 2500 N Fort Valley Road Bldg #1 Flagstaff, AZ 86001, at least 10 days prior to the event. Applications can be faxed, mailed or dropped off in person. Payment can be made by personal check, money order, cashiers check, cash, or over the phone with a credit card (928-226-2741). Both the application and license fee must be received by the deadline. If the application is received after the 10 day deadline a $20.00 penalty fee will be charged. If the application is received the day of the event a $60.00 penalty fee will be charged.
Fee Schedules which may apply to Raw Spirit Fest Vendors
* Non-profit food operations and samplers $65.00
* For profit food at a non-profit event; vendors that sell non-potentially hazardous foods (lemonade, etc.) $95.00
Penalty fee for applications received less than 10 days before the event $20.00
Penalty fee for applications received at the event $60.00
